  • Assist clients with personal hygiene, including shower, tub or bed baths, oral care, hair and skin care.

  • Assist clients in the use of toilet facilities, including bed pans.

  • Assist clients in and out of bed, with the use  of a  mechanical lifting equipment aide is trained and documented as competent.

  • Assist clients with walking, including the use of walkers and wheelchairs, when applicable.

  • Give clients a reminder to self-administer there medications.

  • Meal preparation and feeding, when required.

  • Assist with prescribed exercises when the client and the aide have been instructed by the appropriate health professional.

  • Provides patients’ personal hygiene by giving bedpans, urinals, baths, back rubs, shampoos, and shaves; assisting with travel to the bathroom; helping with showers and baths.

  • Provides for activities of daily living by assisting with serving meals, feeding patients as necessary; ambulating, turning, and positioning patients; providing fresh water and nourishment between meals.

  • Provides adjunct care by ice packs, heat treatments, sitz and therapeutic baths.

  • Maintains patient stability by checking vital signs and weight; recording when intake and output information when instructed.

  • Provides patient comfort by utilizing resources and materials; transporting patients; answering patients’ requests; reporting observations of the patient to home care nurse.

  • Documents actions by completing forms, reports, logs, and records.

  • Maintains work operations by following policies and procedures.

  • Protects organization’s value by keeping patient information confidential.

  • Serves and protects the agency by adhering to professional standards, agency policies and procedures, federal, state, and local requirements, and Virginia standards.

  • Updates job knowledge by participating in educational opportunities; reading professional publications; participating in professional organizations; maintaining license.

  • Enhances agency reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

  • Must have a High School Diploma or GED.

  • Required to have State Certifications for the position.

  •  Monitor, record, and report symptoms or changes in patients’ conditions.

  • Maintain accurate, detailed reports and records.

  • Record patients’ medical information and vital signs.

  • Modify patient treatment plans as indicated by patients’ responses and conditions.

  • Monitor all aspects of patient care, including diet and physical activity.

  • Instruct individuals, families, or other groups on topics such as health education, disease prevention, and development of condition.

  • Maintains work operations by following policies and procedures.
